In part one of Return to Mysterious Island, the story ends with the unlocking of the shield that isolated the island from the rest of the world. In this second installment it all begins with the crash of the helicopter that came to rescue Mina: a sudden volcanic eruption brings down the aircraft. Mina survives but is once again trapped on the beautiful island. All of a sudden, under her very eyes, an unknown disease begins to afflict the plants, then the animal life. Mina explores parts of the island she never visited in order to locate its source. She searches Captain Nemo's base and stumbles upon the ruins of an unknown civilization. Will she find the cure to the disease that is threatening the entire eco-system on the island?

- Chat module: This involves the integration of a chat module (text and direct communication over the internet in the game via a separate and independent interface.
- Two-character play: Mina adopts a cute, little monkey, jep, with whom the player can also play in order to reach places inaccessible to mina, or simply to communicate with other monkeys.
- Scoring system: In the game, each of the player's actions earns them points. One soon discovers the pleasure of restarting the game several times in order to beat one's own record, or that of someone they know.
